REMINDER FOR ALL 5TH GRADERS

To the Parents/Guardian of: ALL 5TH GRADE STUDENTS:

 

            All students entering the sixth grade in August 2015 will be required by law to have a sixth grade physical and an updated immunization certificate.

 

The Department of Education issued new guidelines for school physicals and immunizations.

·         School Physicals must be filled out on the new Kentucky Preventative Health Care Examination. (Health care providers should have a copy of the forms). The physical will be valid for next year, if gotten anytime this year…

·         Immunization Certificates MUST be on the new Kentucky State Form. ALL DATES ARE REQUIRED FOR THE CERTIFICATE TO BE ACCEPTED. We will not accept any certificates that have single dates on them.

 ·         If your child will be 11 or 12 years of age and will be enrolling in the 6th grade in August 2015, a Tdap, 2nd Varicella (Chicken Pox), and a Meningitis vaccine is required for your child, if he/she has not received it.
